Since antiquity, the Unani system of medicine has been participating in health care system. Usually, four modes of treatment viz. regimenal therapy, dietotherapy, pharmacotherapy and surgery are applied for the treatment of diseases. Regimenal therapy is an important mode in which the morbid matter present in the body is either dispersed/excreted or its unnecessary production is blocked or its flow is restricted and the diseases are cured by natural healer of the body, consequently bring back the humoural stability. Nearly 30 regimens have been mentioned in classical Unani literature. Commonest regimenal procedures such as fasd (venesection/phlebotomy), hijāma (cupping), ta‘līq al-‘alaq (hirudotherapy/leech therapy), ishāl (purgation), qay’ (emesis), idrār - i-bawl (diuresis), huqna (enema), ta’rīq (diaphoresis), riyādat (exercise), dalk (massage), hammām (bathing), tadhīn (oiling), natūl (irrigation), sakūb (douching/spraying), inkibāb (steam/vapour application), takmīd (fomentation) etc. are usually applied for the management of various ailments. These regimenal procedures are completely based on holistic approach and are potential but needs to be explored scientifically. This review outlines the therapeutic applications of various regimens of regimenal therapy used in Unani medicine.

In Unani System of Medicine, humours ( Akhlat ) play a vital role in maintenance of health; imbalance of their proportion either qualitative or quantitative can cause disease. Akhlat refers to the body fluids under a wider perspective in the Unani Medicine, which also encompasses fluids designated as hormones by the Modern Medicine. Akhlat (humours) are those moist and fluid parts of the body which are produced after transformation and metabolism of the nutrition; they serve the function of growth, repair, produce energy, preservation of individual and the species. It is concerned with growth, multiplication, differentiation, and metabolic activities of different tissues and systems, and thereby maintains a correct physiological balance between them. Present paper is a review on the concept of Akhlat and an attempt to understand hormones.

The pathogenesis and clinical characteristics of Humma-e-Wabai were described several years ago in the Unani System of Medicine close to the clinical manifestation associated with epidemic or pandemic situations. In the Unani System of Medicine, Humma-e-Wabai described under the legend of epidemic disease ( Amraz-e-Wabai ). Amraz-e-Wabai is an umbrella term which is applied for all types of epidemic or pandemic situation. Renowned Unani Scientists like; Zakariya Rhazi (865–925 AD), Ali Ibn Abbas Majusi (930–994 AD), Ibn Sina (980–1037 AD), Ismail Jorjani (1,042–1,137 AD), Ibn Rushd etc., explained that Humma-e-Waba is an extremely rigorous, lethal fever, that is caused due to morbid air (fasid hawa) and it frequently spreads among the larger population in the society. There are four etiological factors responsible for Amraz-e-Wabai viz ; change in the quality of air, water, earth, and celestial bodies, which was described by Ibn Sina in Canon of Medicine. He also advised that movements should be limited during epidemic situations. Shelters should be fumigated with loban ( Styrax benzoin W. G. Craib ex Hartwich.), Kafoor ( Cinnamomum camphora L.), Oodkham ( Aquilaria agallocha Roxb .), Hing ( Ferula foetida L.), myrtle ( Myrtus communis L.), and sandalwood ( Santalum album L.), etc. The use of vinegar ( sirka ) and rose water ( arque gulab ) has been advocated to prevent the infection by spray. Avoid consumption of flesh, oil, milk, sweets, alcohol. Food prepared with vinegar. Specific antidotes (e.g. Tiryaq-e-Wabai, Tiryaq-e-Farooque ), should be used as prophylaxis. This review attempts to explain the concept, prevention, and management of epidemic or pandemic situations.

Emergence of antimicrobial resistance, with practically all newly developed and even more potent antibiotics like carbapenem and colistin, has become a matter of great public health concern. A number of common diseases of public health importance are becoming harder and sometimes impossible to treat due to increased resistance of pathogens. These alarming threats seek prime attention of scientific community to develop newer antibiotics with long-lasting efficacy, least side effects, and low economic burden. Unani classical texts have enormous citations on different infectious diseases. Many single drugs and compound formulations are in vogue since ages for the treatment of infectious diseases. Use of Afaviya (spices), Mufarrehat (exhilarants), Tiryaqi Advia (drugs with antidote properties) and Sirka (vinegar) is highly advocated in treating various infectious diseases. Though, enormous research activities have been undertaken worldwide to explore and develop newer antibiotics from natural resources, indicating massive magnitude of natural products in treating various infectious diseases, however, the higher plants, still largely seems to be unexplored. Moreover, elaborated and well controlled clinical studies are still lacking to authenticate their clinical significance. Hence, a rigorous, well-designed & well-structured research is highly paramount to ascertain the provision of newer, relatively safe and cost effective natural antibiotics.

Podophyllum hexandrum Royle [= Sinopodophyllum hexandrum (Royle) T.S. Ying] is an important, endemic medicinal plant species of Himalaya. It is used in Unani System of Medicine under the name of ‘ Papra’ . The drug was not mentioned in previous literatures, but the first time it introduced in Unani Medicine by a great scholar Hakim Najmul Ghani. He has mentioned its uses and benefits in his classical book Khazainul Advia . In Unani Medicine the plant species has been used to treat various ailments like constipation, fever, jaundice, liver disorders, syphilis, diseases of lymph glands etc. In Kashmir Himalaya it is used to treat various diseases by local medicinemen, but now it is listed in rare drugs. Various pharmacological studies have been done such as antioxidant, antimicrobial, anti-inflammatory, antifungal, radio-protective etc., recently it has also been reported that podophyllotoxin or podophyllin can be used to treat some forms of cancers also.

Objectives This study examined effect of berberine and piperine on neuroprotective potential of neostigmine in the management of neurological disorders. Methods Berberine and neostigmine were weighed (30 g), dissolved in distilled water (30 mL) separately, while, 30 mg piperine was dissolved in ethanol (0.45 mL), made up to 30 mL with distilled water. Antioxidant activities in 2, 2-diphenyl-1-picrylhydrazyl radical (DPPH), 2, 2-azinobis (3-ethylbenzothiazoline-6-sulfonate) radical (ABTS), Fe-chelation, ferric reducing properties (FRAP), nitric oxide (NO) and hydroxyl (OH) radical scavenging abilities and Fe 2+ , cisplatin and sodium nitroprusside (SNP) induced lipid peroxidation (LPO), and acetylcholinesterase (AChE), butyrylcholinesterase (BChE) and monoamine oxidase (MAO) activities were assessed in vitro. Results The result revealed that tested compounds inhibited enzymes activities dose-dependently. However, berberine (IC 50 =0.17 mg/mL) had slight higher AChE inhibitory effect than piperine and neostigmine (p<0.05). Also, berberine had the highest BChE inhibitory effect (IC 50 =0.16 mg/mL) while piperine exhibited the highest MAO inhibitory effect (IC 50 =0.21 mg/mL). Berberine, piperine and neostigmine exhibited high antioxidant properties and inhibited Fe 2+ , cisplatin and SNP induced LPO . Conclusions Both alkaloids demonstrated antiradical scavenging ability comparable to neostigmine action against Alzheimer’s disease (AD). The modulatory and antioxidant berberine and piperine properties on these enzymes (AChE, BChE and MAO) could be possible underlying mechanisms in employing these compounds as a complementary therapy in neurodegenerative diseases (NDDs) management.

Objectives Neera , non-fermented coconut inflorescence sap (NFCIS) and its fermented form, toddy (FCIS) are the two well-known traditional drinks in South India. Both the saps show high rejuvenation effect and NFCIS is used for the curing of bronchial suffocation, anemia, tuberculosis and piles in traditional medicine. But, a few scientific studies have been reported on their health benefits so far. Presently, the antioxidant effect of both non-fermented (NFCIS) and fermented form (FCIS) of coconut inflorescence were analyzed in experimental animals. Methods The free radical scavenging property of FCIS and NFCIS was analyzed in vitro . The effect of these saps on mitigating sodium fluoride (NaF) deteriorated redox status was evaluated in mice. Results NFCIS exhibited high antioxidant activity than its fermented form. NFCIS reduces metal ions and scavenge hydroxyl and DPPH radicals with IC 50 values 6.5 ± 1.9 and 44 ± 3.14 µL/mL, respectively. Supplementation of NFCIS for 14 days increased SOD, CAT and GPx activities and GSH level in liver by 51.67, 52, 27.88 and 35.77%, respectively against NaF induced decline with a concomitant decrease in lipid peroxidation to 40.76%. Saps rich in minerals indicate pharmaceutical and nutritional value. Conclusion The study revealed the antioxidant efficacy of non-alcoholic natural drink, Neera and recommends an alternative for synthetic carbonated soft drinks. The regular consumption of Neera may protect the body from various chronic diseases especially where the oxidative stress played as a key role.

Objectives Ghee is widely considered as the Indian name for clarified butterfat and processing of ghee with therapeutic herbs i.e., ghrita is renowned for augmenting their medicinal properties. The wound is considered as a challenging clinical problem with early and late complications. To reduce the burden of wounds with the shortest period and minimum scaring, an attempt was made to prepare and evaluate the wound healing potential of ghee based polyherbal formulation. Methods Based on local ethnic tribal claims, Semecarpus anacardium L., Argemone mexicana L., Cocculus hirsutus L., and Woodfordia fruticosa K. were collected from Western Ghats of India. The polyherbal Bhallatakadi Ghrita (BG) formulation was prepared as per Ayurvedic procedure and assessed for its wound healing potential using incision and excision wound animal models. Results BG treated group showed a complete contraction of wounds (99.82 ± 0.10%) (p<0.001) with 15.17 ± 0.40 days re-epithelization time and breaking strength (531.50 ± 5.89) (p<0.05). The hydroxyproline content of BG was found to be significantly higher i.e., 4.23 ± 0.21 (p<0.05). Quantitative estimation of BG exhibited 54.7 ± 3.7 mg100 g −1 of polyphenols and 42.3 ± 5.4 mg.100 g −1 flavonoids in terms of gallic acid and quercetin respectively. HPLC analysis revealed the presence of gallic acid and quercetin whereas the presence of fatty acids was confirmed by GC-MS analysis. Conclusions It may conclude that the presence of quercetin, gallic acid, and fatty acids could have accelerated the healing rate of the ghrita formulation, as they have already been known for their potential wound healing properties.

Objectives Tuberculosis continues to be a major public health problem globally, despite incredible advancements in healthcare system. In Unani system of medicine, Qurs Tabasheer Sarthani (QTS) and Arq Hara Bhara (AHB) have been traditionally used for tuberculosis like conditions. The study was aimed to investigate the effects of co-administration of QTS and AHB with category I first line antitubercular drugs (CAT-I) on the indices of liver and kidney function in rats. Methods QTS and AHB were prepared individually and mixed to achieve final compound Unani pharmacopoeia formulation (UPF). The human equivalent doses for rats were calculated and administered with and without CAT-I. The effects of the formulations on serum indices of kidney and liver function, hematological markers and plasma CAT-I drug levels were estimated at 14th, 60th & 180th days of treatment. Results The administration of UPF, CAT-I and UPF + CAT-I altered the levels of aspartate transaminase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP) and gamma glutamyltransferase (GGT) and haematological markers. These alterations were within permissible range and randomly distributed among groups during various time points. Administration of CAT-I alone resulted in moderate histopathological changes which were completely abrogated in CAT-I + UPF co-administered animals. The co-administration of UPF with CAT-I improved the plasma peak rifampicin (RIF) levels, without altering the liver and kidney functions. Conclusions The co-administration of UPF with ATT improved liver and kidney functions and increased the plasma levels of RIF. These beneficial findings provide a scope to evaluate the pharmacokinetic studies in humans.

Background Exposure to organic solvents (OS) adversely affects different body systems, the central and peripheral nervous systems being the most susceptible ones. Objectives This study investigated the role of iron in association with some neurotransmitters for diagnosis of neurotoxicity of OS. Methods The study included 90 workers, 50 occupationally exposed to OS and 40 representing control group. Blood samples were collected from the included subjects for determination of serum iron, total iron binding capacity (TIBC), serotonin and gamma-aminobutyric acid (GABA). Results Revealed reduction in serotonin level and serum iron. However, the elevation in GABA and TIBC was observed. The duration of exposure was significantly correlated with iron and serotonin while it was positively correlated with GABA and TIBC. Conclusions Elevated GABA and TIBC with decreased serotonin and serum iron can be used as early diagnostic measures to detect the neurotoxic effects of OS.

Objectives There is a growing body of evidence indicating the potential of culinary herbs and spices to decrease the incidence of several chronic diseases or conditions. Because of this, the WHO recommends their regular consumption. In the Cameroonian culinary practices, “Nkui” is a famous dish made from a mixture of 10 spices. In our previous study, the ethanolic extract of this mixture exhibited promising estrogenic properties. Thus, this study aimed to evaluate its protective effects on some menopause-related cardiovascular and bone disorders. Methods For this purpose, a post-menopause-like model (ovariectomized rats) has been used. Animals were orally treated with the “Nkui” extract for 60 days. The investigation focused on the oxidative stress status, endothelial function (NO bioavailability), lipid profile, and bone mass, biochemical (calcium and inorganic phosphorus contents, serum alkaline phosphatase activity) and histomorphological features. Results The extract regulated lipid metabolism in a way to prevent accumulation of abdominal fat, gain in body weight and increased atherogenic indexes induced by ovariectomy. It prevented menopause-related low levels of nitric oxide and oxidative stress damage by increasing superoxide dismutase and catalase activities, while reducing glutathione and malondialdehyde levels in the heart and aorta. Moreover, it prevented ovariectomy-induced bone mass loss, bone marrow disparities and the disorganization of the trabecular network. It also increased femur calcium and inorganic phosphorus contents. Conclusions These results suggest that a regular consumption of “Nkui” may have health benefits on cardiovascular system and osteoporosis, major health issues associated with menopause.

Objectives This study adds to the geography of complementary and alternative medicine (CAM) literature by comparing the spatial-temporal patterns of five types of CAM within 19 cities in light of clustering benefits from localization economies. Methods CAM office location points and nearest neighbour, standard distance, local spatial autocorrelation, and Mann–Whitney analyses are utilized to test potential clustering tendencies of CAM types over time. Results It is shown that ‘within’ (chiropractors near chiropractors, for example) and ‘amongst’ (chiropractors proximate to other CAM types) spatial clustering occurs in 2007 and 2017. This implies the persistent influence of localization economies. Conclusions Continued clustering of CAM within urban locations already replete with CAM offices will widen spatial disparities through time. This has implications for policy-makers concerned with dispersing medical resources over space for better accessibility.

Objectives Stroke survivors dissatisfied with their progress often seek complementary and alternative interventions (CAI). This paper reviews the evidence for CAIs in stroke recovery. Methods A literature search was performed for publications until December 2019 of CAI for stroke in Pubmed, Cochrane Library, EMBASE, CINAHL, AMED. Evidence was assessed according to Oxford Centre for Evidence-based Medicine criteria. Results In a meta-analysis, acupuncture reduced death or dependency compared to control at the end of follow-up and over the long term (≥3 months), OR 0.61(95%CI 0.46–0.79) and OR 0.67(95%CI 0.53–0.85) respectively, but was neutral against sham acupuncture. A Cochrane review of acupuncture vs. sham acupuncture in subacute or chronic stroke vs. showed no differences in motor function and quality of life. Three trials found favourable effects of moxibustion on motor function (SMD=0.72, 95%CI 0.37–1.08, p<0.0001). Two trials showed cupping compared to acupuncture reduced hemiplegic shoulder pain and upper-limb ‘myodynamia’. A meta-analysis of traditional Chinese medicines for ischaemic stroke showed marked improvement in neurological deficit on stroke scales. There was no evidence for Ayurveda , homoeopathy or reiki . Tui-na reduced the Modified Ashworth Scale in some muscle groups. Marma massage improved Motricity Index and trunk control. Thai massage and herbal treatments improved the Barthel Index. On meta-analysis, Yoga improved memory and anxiety, while tai-chi improved activities of daily living, balance and walking ability. Studies were generally of poor quality. Conclusions The evidence for benefit of CAIs for stroke recovery is weak. More research is needed to justify these treatments for stroke, by well-conducted, adequately-sized, double-blinded, randomized controlled trials.

Objectives The global epidemic of overweight and obesity presents a major challenge in the health status of the society. Their prevalence is at an alarming rate worldwide due to poor compliance with conventional treatment and high rates of relapse, thus increasing demand for an effective and safe alternative approach such as hypnotherapy. This study aimed to evaluate the feasibility and acceptability of hypnotherapy for weight loss and to compare these among selected socio-demographics. Methods A cross-sectional study was conducted among 30 students and staff of a public university in Terengganu, Malaysia using convenience sampling. Data analysis was carried out using SPSS 23.0. Results Among the respondents (age = 26.17 ± 8.23 years; female = 66.7%; students = 63.3%), 40.0% were overweight and 60.0% were obese. Results indicated excellent feasibility as determined by participants’ satisfaction towards the clarity of hypnotherapist’s voice (93.3%), the suitability of content (86.7%) and time spent for the session (90.0%). Good overall acceptability (>60.0%) was also reported regarding hypnotherapist professionalism, the environment and perceived usefulness of hypnotherapy. Obese individuals were significantly more satisfied towards the hypnotherapist environment than overweight respondents (p=0.015). Additionally, no adverse effects were reported after the intervention. Conclusions This evidence signalled that hypnotherapy is a promising alternative tool in assisting overweight and obese individuals to lose weight. Extensive research is needed to substantiate its role in weight management programs for its full benefits.

Background Chronic migraine is a disabling condition that negatively affects many aspects of migraineurs’ lives. Patients who use pharmacological agents experience various side effects. Some studies have reported clinical improvement following wet cupping therapy in migraine patients. This study examined the effectiveness of wet cupping therapy on headache severity and its complications in patients with migraine. Methods In this study, a total of 29 patients previously diagnosed with migraine were treated with wet cupping therapy and followed up for headache severity. The severity of pain was measured before therapy, 1 h after therapy, and one month after therapy. The wet cupping therapy was performed by an experienced nurse who was not part of the research team. Results The mean age of the patients was 36.17 (Sta. Deviation: 10.74 years; range: 22–68 years), and on average, patients had experienced migraine for 11.14 years. The severity of headache pain was decreased significantly from 6.66 (very intense pain) before cupping therapy to 2.48 (discomforting pain) 1 h after cupping therapy and 0.72 (very mild pain) one month after cupping therapy (p<0.001). The observed complications were skin pigmentation (6.9%), skin scarring (24.1%), and itching in the cupping region (31.0%). Conclusions The present study suggests a significant improvement in the severity of headache in migraine patients 1 h and one month after wet cupping therapy.

Objectives Many women in recent years have been willing to undergo puncture surgery for childbirth, which, like other surgeries, has physical and psychological side effects such as incision, infertility, chronic pain, and anxiety. Therefore, it is important to reduce and improve these side effects. The aim of this study was to determine the effect of foot massage with orange essential oil on anxiety in women undergoing cesarean section. Methods This randomized clinical trial study was conducted in 2019 on 80 women referred to Bahar Shahroud Hospital after cesarean section surgery. The samples were divided into two groups by intervention (foot massage with orange essential oil) and control (foot massage without orange essential oil). The Spielberger scale was used to determine anxiety scores after cesarean section. In the intervention group, the feet were massaged with orange essential oil, and in the control group, the orange essential oil massage was performed without oil. Anxiety before, immediately after, and 60 min after the intervention was measured and evaluated in both groups. Data analysis was performed using descriptive and inferential statistics. Significant levels were considered for all statistical tests (p<0.05). Results The anxiety score before the intervention in the two groups of intervention and control was 57.12 ± 3.12 and 57.07 ± 3.54, respectively, which were not significantly different, but immediately after the intervention, the anxiety scores in both groups decreased significantly so that there was a further decrease in the intervention group (52.10 ± 4.75 and 56.02 ± 3.77), 1 h after the intervention, the anxiety score in the intervention group decreased compared to the previous stage and increased in the control group (50.40 ± 3.74 and 56.85 ± 4.27). Conclusions Foot massage with orange essential oil can probably be effective as a proper nursing intervention in reducing anxiety after cesarean section surgery.

Objectives Hectic, late-night lifestyle has reduced 90 min sleep in 20% adults resulting in insomnia and excessive daytime sleepiness (EDS). We assess the scope of Sudarshan Kriya Yoga (SKY), a 4-component, breathing process in reducing EDS, generally and situationally. Methods This is a prospective, controlled study involving randomized subjects without any sleep-wake cycle anomalies and prior experience in SKY. Subjects (n=52) performed 30 min of SKY for 6 days/week for 8 weeks, while controls (n=53) performed sitting activity and Suryanamaskar for 4-weeks each. Epworth Sleepiness Scale (ESS) was used to measure EDS at 0, 4, and 8 weeks. Results SKY group showed significant ESS score improvements between 0–4 weeks and 4–8 weeks of 1.22 (p=0.0001) and 1.66 (p=0.001) respectively. Controls however failed to improve with score differences of 0.02 (p=0.892) and 0.02 (p=0.8212) respectively. SKY group showed significant ESS score improvement over controls at 4-weeks (difference=1.74; p=0.013) and 8-weeks (difference eight; p=0.0001). Improvement was most for obese people and those sitting in a halted car. Conclusions Improvement in subjects’ nighttime sleep and daytime wakefulness in SKY practitioners can be attributed to polyvagal theory. Increased heart rate variability (HRV) alterations and sympathetic hyperarousal in chronic insomnia; and cholinergic and GABAergic dysregulation in anxiety disorders are countered by regulated vagal nerve stimulation post SKY. Our study establishes effectivity of SKY in reducing EDS (total and situational), provides a clinical correlation for prior polysomnographic evidence and paves way for larger trials directed towards SKY prescriptions for insomnia.

Objectives The objective of the study was to compare traditional resistance, instability resistance, and plyometric training methods on measures of athletic performance in healthy subjects. Methods Sixty five healthy, physically active male students were randomly assigned to the following groups: traditional resistance training (RT, n=22), instability training (IT, n=22), and plyometric training (PLY, n=21).Athletic performance parameters were assessed pre- and post-training using chair squat test, standing stork test, shuttle run test, t-test, and vertical jump test. Results General linear univariate model with baseline as covariate (ANCOVA) was used for analyzing the change in outcome from baseline to post-treatment. Statistically significant improvement was observed in all the athletic performance parameters in all three groups after seven weeks of training (p-value<0.05). The highest change in chair squat test was reported for RT compared to IT (p-value<0.001) and PLY (p-value<0.001). The change in standing stork test among the IT group was substantially higher than that among RT (p-value=0.007) but did not significantly differ from that among the PLY (p-value=0.27). No statistically significant difference was observed in post-test values of vertical jump test among three groups. The highest change in t-test and shuttle run test was reported for PLY compared to IT (p-value<0.001) and RT (p-value<0.001). Conclusions Based on the findings of this study, it is suggested that IT and PLY can be included with traditional RT to improve various aspects of athletic performance in healthy physically active individuals. The current study will give an insight to athletes, coaches, and trainers regarding utilization of appropriate training methods in enhancing athletic performance. However, further research is required to establish the effectiveness

Objectives The quality of management of hyperuricemia has remained sub-optimal owing to unavoidable toxicities, limitations, and dearth of novel advances. Homeopathy has remained under-researched in hyperuricemia. We investigated the clinical effectiveness of three treatment regimens – individualized homeopathy (IH), Urtica urens mother tincture (UUMT), and both (IH + UUMT) along with lifestyle modifications in a sample of 90 patients with hyperuricemia. Methods An open, randomized (1:1:1), 3 parallel arms (IH, UUMT, and IH + UUMT), pragmatic trial was conducted at National Institute of Homoeopathy, Kolkata. Outcome measures were serum uric acid (primary), Gout Assessment Questionnaire version 2 (GAQ2, secondary), and Measure Yourself Medical Outcome Profile version 2 (MYMOP2, secondary); all measured at baseline, and after 3 and 6 months. Intention- to-treat sample was analyzed to detect group differences by unpaired t tests. Results Attrition rate was 8.9% (IH: 3, UUMT: 3, IH + UUMT: 2). Groups were comparable at baseline. Reductions in serum uric acid over 3 months were comparatively higher (p=0.057) in the UUMT group than others, however, the differences were narrowed over 6 months (p=0.119). Per protocol analysis of serum uric acid level revealed similar trend of significantly higher reduction in the UUMT group than the other two (3 months: p=0.001; 6 months: p=0.007). No significant differences existed in reductions of GAQ2 scores among the three groups. Few significant differences were detected in MYMOP scores over 3 months favoring IH against others (symptom 2, p=0.001 and wellbeing score, p=0.002), and also over 6 months favoring IH + UUMT against others (symptom 1, p<0.001). Conclusion Although all three therapies showed similar improvements, the IH + UUMT group had more positive direction of effects than IH or UUMT alone; however, no definite conclusion could be arrived at. Further trials are warranted with larger sample size. Trial registration: CTRI/2018/05/014026.

Objectives Exposure to wood dust may lead to impairment of the lung functions. Microsomal epoxide hydrolase enzyme (EPHX1) was shown to take part in protection against oxidative stress. An alteration in enzyme activity might be associated with its gene polymorphisms. In vitro polymorphisms in exons 3 (His113Tyr) and 4 (Arg139His) lead to reduced activity (slow allele) and increased activity (fast allele). Macrophage inflammatory protein 2 (MIP-2) is produced in rat lung epithelial cells after exposure to fine particles. We aimed to investigate the associations between mEPHX1 polymorphisms (in exon 3 and 4) and lung function in furniture workers and assessment of MIP-2 effect. Methods Our study was performed on 70 wood dust exposed male workers and 70 matched normal controls subjects. Ventilatory function tests were measured by spirometer, MIP-2 was performed by ELISA methods and EPHX gene was done by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) methods for each participant. Results Significant reduction in forced vital capacity (FVC%) and forced expiratory volume in the first second (FEV1) levels in Tyr–Tyr and Tyr–Hist genotypes of EPHX (exon 3) was observed. Reduced peak expiratory flow (PEF) levels and significant rise in MIP-2 levels were detected in Tyr–Tyr genotype. While high significant reduction in FVC% and FEV1 levels were shown in different genotypes in exon 4. Significant rise was observed in MIP-2 levels in Hist–Hist genotype of exon 4. An increase in duration of exposure showed positive correlation with fall in ventilatory functions. Conclusions It was concluded that in Hist139Arg of EPHX gene, fast genotype (Arg–Arg) was associated with impaired ventilatory functions.

Objectives Spinal glycinergic mechanisms inhibit the micturition reflex, and administration of glycine inhibits bladder activity in rats. Therefore, we examined whether dietary glycine would improve storage symptoms in urological outpatients. Methods We enrolled 20 participants (16 men and four women) with an overactive bladder symptom score (OABSS) ≥ 3. All participants took 3 g of glucose (placebo) twice a day for the first four weeks, then 3 g of glycine twice a day for the next four weeks. We evaluated blood pressure, international prostate symptom score (IPSS), nocturia quality of life (N-QOL) score, OABSS, frequency of urination, sleep latency, time to first nighttime void, bladder pain, global self-assessment (GSA) evaluated urinary symptom improvement, and adverse events. Results Glucose administered as a placebo improved urinary frequency, urine force on the IPSS, and five of the 13 items on the N-QOL. However, compared to the results before and after glucose administration, glycine treatment decreased the number of nocturnal voids, urgency, and total score for urine storage items on the IPSS. It also reduced blood pressure and improved IPSS-QOL. For the OABSS, improvements with glycine were noted in the number of nocturnal urinations, urinary urgency, urge incontinence, and total score. For the N-QOL, eight of 13 items, and the total score, improved. The actual number of nighttime urinations, sleep latency, latency to first nighttime urination, bladder pain, and GSA also improved. There were no adverse events. Conclusions Glycine might improve urine storage symptoms, cardiovascular function, pain, and sleep.

Objective The aim of this study was to investigate the effect of coenzyme Q10 (CoQ10) supplementation on oxidative stress engendered from hypoxia in population live at high altitude. Methods This is an intervention study in which 50 females of volunteers population-36 of them who live at high altitude compared with the placebo group (14 from the total population that live at sea level). Blood samples were collected in -anticoagulant tubes from control and high altitude before and after CoQ10 supplementation (150 mg/day for 2, 4 and 8 weeks). Plasma was separated and used for the determination of malondialdehyde (MDA), nitric oxide (NOx), total antioxidant capacity (TAC), paraoxonase (PON1) by spectrophotometer, CoQ10 and vitamin E by high performance liquid chromatography (HPLC). Results Our results appeared that TAC, PON1, vitamin E and CoQ10 concentrations were significantly decreased in population at high altitude at base line compared to placebo group population at sea level. Whereas, administration of CoQ10 attenuated all measured parameters especially after eight weeks of administration. Conclusion We concluded that coenzyme Q10 supplement at a dose of 150 mg/day has a powerful effect in oxidative stress parameters and increased antioxidant parameters included vitamin E in population with hypoxia after 4 and 8 weeks. So that supplementation positively affects oxidative stress and is recommended CoQ10 supplementation in population who live at high altitude.

Objectives Terminalia arjuna (TA) has been widely used as folk medicine since ancient times. Apart from its therapeutic properties it also has anti-spermatogenic activity, but its effectiveness and mode of action on male gonadal activity remains to be revealed. Methods TA bark extract was dissolved in 1% dimethyl sulfoxide and applied in primary Leydig cell culture from rat testis in dose dependent manner. After 24 h of treatment cellular toxicity marker, cytokines, steroid 5-alpha-reductase 1 (SRD5A1) and androgen receptor (AR) were measured. Results Present study first proposed the cytotoxic impacts of TA on Leydig cells via leakage of lactate dehydrogenase and inflammatory responses by altering the pro- and anti-inflammatory cytokines in dose-dependent-manner. Furthermore, down regulation of SRD5A1 and AR indicated inhibition of normal steroidogenesis which well corroborated with cytotoxity and inflammation related parameters to promote damage of Leydig cell function. Nevertheless, it can be speculated that the inhibition of normal steroidogenesis and thereby spermatogenesis induced by TA is possibly due to the presence of β-sitosterol. Conclusions Therefore, the dose and duration of treatment should be carefully monitored in cases of its medicinal uses and longer duration should be avoided to protect reproductive health and fertility.

Objectives Reduction in cognitive function of college students affects their academic recitation. Acupuncture is widely practiced for health promotion. Objective of this study was to evaluate the effect of acupuncture on cognitive task performance in college students. Methods Sixty students aged 18–25 years were randomly allocated into acupuncture group (AG) (n=30) and control group (CG) (n=30). The AG underwent 20 min of acupuncture/day, while the CG underwent their normal routine for 10 days. Assessments were performed before and after the intervention. Results Between-group analysis showed a significant increase in the six-letter cancellation test (SLCT) score in AG compared with CG. Within-group analysis showed a significant increase in the scores of all tests (i.e. SLCT, forward and backward Digit span test [DST]) in AG, while a significant increase in backward DST was observed in CG. Conclusion Acupuncture has a beneficial effect on improving the cognitive function of college students.

Objectives Covid-19 Pandemic has affected everyone’s mental health. In addition to several preventive measures such as wearing a mask, using sanitizer, measures also need to be taken to prevent anxiety and depressive disorders due to this unexpected crisis situation. Practicing yoga is one of the simple, scientific methods to combat stress and prevent anxiety among children. Methods The scientific evidence and anecdotal experiences on benefits of yoga is described in this paper, highlighting the importance of yoga in nurturing the mental well-being in children. Results Scientifically designed and conducted studies as part of the research programs by health professionals objectively conclude that mental health parameters improve significantly with yoga as an intervention. In addition to mental health, yoga will also improve the physical health and boost immunity among children which will also help in reducing the infection rate in children. Conclusions As a way forward, authors strongly recommend establishing yoga as a curriculum at scale to cover the vast vulnerable population of young children who are the future of the nation.

Objectives A healthy lifestyle is one, where the individual adapts it aiming at prevention from ailments. Unhealthy lifestyle is a behavioral risk factor for non-communicable diseases (NCDs). Arogya Raksha Panchatantra (five principles of healthy living), is a Naturopathy and Yoga lifestyle practice proposed by an eminent Indian Naturopath Dr. B. Venkat Rao, observed to modify the behavioral risk factors. The main objective of this study is to observe the changes in Hematological parameters and Anthropometric measures by practicing this Naturopathic lifestyle. Methods The study adapted one group Pretest – Posttest quasi experimental design, with a total of nine healthy student volunteers between age group 18–20 years. Hematological parameters such as total blood cells count, hemoglobin concentration, MCV, MCH, PCV, and anthropometric measurements such as weight, body circumferences (waist, hip) and changes in blood pressure were measured at baseline and after practice at 25th day. Results There was a statistically significant difference observed in weight, waist and hip circumference, hematological indices except with MCV and MCH, and blood pressure (p<0.05), with no significant changes in waist-hip ratio and blood counts. Conclusions The findings of the study indicate that adapting Naturopathy and Yoga lifestyle based on proposed lifestyle practices may be beneficial in reducing the risk factors for non-communicable diseases. The study does not attribute the effects observed to any particular lifestyle practiced in this study; rather it is combination of healthy practices as observed in the study and it needs further longitudinal observations whether the beneficial effect of Naturopathy lifestyle practices is sustained for longer period of time.

Objective The purpose of this case report was to create awareness among scientific fraternity and those who are in the regular usage of the yellow phosphorous compound. Yellow phosphorus is an inorganic element that is poisonous to humans (yellowish, waxy, crystal-clear solid with a pungent odor). Case presentation The patient treatment chart was reviewed and the patient’s condition was evaluated daily. Consumption of toxic doses causes shock and cardiovascular failure within 2–3 h of ingestion, and peak levels are reached when distributed to all tissues after absorption. Here, we report a case of self-harm with the ingestion of rat killer paste, and the patient suffered internal bleeding and acute liver failure. Conclusion Rodenticides remain the primary cause of morbidity and mortality, internal bleeding is the major risks of rodenticide poisoning.

Objectives The objective of the study is to evaluate the short-term effect of yoga on plasma glucose, lipid profile, blood pressure and insulin requirement in a patient with type 1 diabetes mellitus (T1DM). Case presentation A 28-years old female was diagnosed with T1DM at the age of seven. She was under Human Mixtard insulin 30-70, thrice/day for 15-15-15 units. She had a history of poor glycemic control, diabetic ketoacidosis and hypoglycemic shock. At the age of 27, she got admitted with complaints of general weakness, joints stiffness and weight loss in our holistic health centre and underwent two weeks of yoga. Results showed a better reduction in the plasma glucose levels, lipid profile, blood pressure and insulin dose and an increase in hemoglobin levels with no episodes of hypoglycemia after yoga. Conclusions Yoga might be considered as a safer and effective adjuvant in the management of T1DM.

Objectives Vipadika (Palmo plantar Psoriasis) is a Kshudra Kushta (minor skin disorder) of Vatakapha origin (body humors like vata and kapha) characterized by Sphutana (cracks) and Teevra Vedana (severe pain) in Pani (palm) and Pada (sole). Based on the symptoms it can be correlated to Palmo Plantar Psoriasis which is a variant of psoriasis which affects the skin of the palms and soles with the features of hyperkeratotic, pustular, or mixed morphologies. The treatment methods for Palmo plantor psoriasis causes severe side effects due to which an evident shift from modern treatments to ayurvedic treatment line. Case presentation A 15 year old, female, with complaint of cracks and dryness over sole of both feet with pain for the last 12 and half years. The case was managed with Padanimajjana (immersion of foot). Padanimajjana is a modified form of Avagaha Sweda (switz bath) where the affected part of the foot is dipped in medicated liquid. Here Mahisha Gritha (Buffaloe ghee) is used for Padanimajjana. Conclusions Mahisha Gritha Padanimajjana shows significant improvement in number of cracks, dryness and pain within short period of time. This is a very simple, economical, and effective treatment for Vipadika.
