Objective
Biomedical Engineering / Biomedizinische Technik (BMT) is a high-quality forum for the exchange of knowledge in the fields of biomedical engineering, medical information technology and biotechnology/bioengineering. As an established journal with a tradition of more than 60 years, BMT addresses engineers, natural scientists, and clinicians working in research, industry, or clinical practice. All articles are peer-reviewed. All manuscripts must be submitted in English.

Starting 2025, the journal will be transferred to Diamond Open Access on a year-by-year basis. All articles will thus immediately appear under the Creative Commons license CC-BY. There will be no publication costs for the authors. The Open Access transformation is based on Subscribe-to-Open, an alternative model that enables the full Open Access transformation of journals through the continuation of existing subscriptions. The prerequisite for successful transformation is that subscriptions are continued to the same extent as before. The Journal was founded in 1956 as the official journal of the German Society of Biomedical Engineering Biomedizinische Technik and published firstly in German. In a transition period as of 2004 publication in German and English was possible, but since 2010 the journal appears only in English. Consistently the title was changed to Biomedical Engineering – Biomedizinische Technik.

Analysis of orthodontic aligner biocompatibility: leachable compounds of different aligner materials

Objectives the aim of this study was to clarify the biocompatibility of different tooth repositioning splints (aligners). This included the characterization of the volatile fraction by headspace solid phase microextraction coupled to gas chromatography mass spectrometry, simulation of leachable organic compounds using artificial saliva and the quantitation of bisphenol A and bisphenol S after extraction. Methods Four different aligners were characterized in this study, these included two splints with novel shape memory properties: a printable aligner made of the resin Tera Harz TC-85 DAC (Graphy Inc., South Korea) and a self-manufactured SMP-Aligner consisting of the components polypropylene carbonate and thermoplastic polyurethane. The other two aligners were conventional, thermoformable aligners: CA® Pro Clear Aligner (Scheu Dental GmbH, Germany) and Erkodur-al (Erkodent Erich Kopp GmbH, Germany). Results no BPA was found in all four samples after 72 h. BPS was found in one Aligner. The most leachable compounds were found in the samples of the SMP- and the direct printed Graphy-Aligner. Most of the compound release occurred during the first 24 h. Conclusions the cumulative exposure effect from aligner wear should be carefully considered, although the current research indicates that the levels of leachable compounds are generally low.

Automated lesion detection in endoscopic imagery for small animal models – a pilot study

Objectives Small animal models, particularly mice, are crucial for studying gastrointestinal diseases like colorectal cancer. Tumor assessment via colonoscopy generates large video datasets, necessitating automated analysis due to limited resources and time-consuming manual review. Methods We employed a YOLOv7-based deep learning model pre-trained on human polyp images to detect tumors in mouse colonoscopy videos. Detection was enhanced using a stool detector and a color-based filter. Lesions were classified from ‘0’ (no tumor) to ‘5’ (tumor >50 % of colon diameter) using a custom ratio-based method. The system was evaluated on 150 videos from 28 mice over 6 weeks, with 125 videos containing tumors. Results Initial detection yielded a Precision of 0.576, Recall of 0.916, and Accuracy of 0.593. Adding the stool detector improved results to 0.932, 0.946, and 0.897, respectively. Compared to expert annotations, classification reached 0.759 Precision, 0.774 Recall, and 0.774 Accuracy over all five classes. Conclusions The proposed approach reliably detects and classifies colon tumors in mice, offering real-time support for preclinical endoscopic studies. Further evaluation will provide more insights into its performance.

Optimized design of porous dental implant structures based on triply periodic minimal surface (TPMS)

Objectives This study aims to optimize the design of porous dental implants using triply periodic minimal surfaces (TPMS) to address stress shielding caused by elastic modulus mismatch between titanium implants and bone tissue, while enhancing osseointegration through controlled porosity. Methods Two TPMS architectures (D-type and G-type) were modeled via MathMod and Rhino software, with porosity controlled by parameter t. Finite element analysis (FEA) evaluated mechanical properties under porosities of 40 %, 60 %, and 80 %, and stress distribution in a patient-specific mandibular model under 200 N masticatory load. Results Both D- and G-type structures met mechanical requirements at ≤60 % porosity. The D-type exhibited optimal stress transfer at 60 % porosity, whereas G-type at 80 % porosity exceeded yield strength due to thin walls. Porous designs reduced cortical-cancellous bone stress differences by 30–50 %, mitigating stress shielding. The interconnected network facilitated bone ingrowth. Conclusions TPMS-based porous implants with 40–60 % porosity balance mechanical strength and stress distribution, offering personalized solutions for bone quality adaptation. Future work should validate long-term stability through in vitro experiments.

Multi-stage image pyramid network for coarse-to-fine segmentation of temporomandibular joint in CBCT images

Objectives Accurate segmentation of the temporomandibular joint (TMJ) in cone beam computed tomography (CBCT) images is crucial for the diagnosis of temporomandibular joint osteoarthritis (TMJ-OA), particularly of the condyle. The aim of this study was to use a multi-stage image pyramid network for precise segmentation of the TMJ in CBCT images. Methods A multi-stage image pyramid network (MIP-Net) was proposed in this study for accurate TMJ segmentation in CBCT images. This network utilizes stepwise down-sampling to form an image pyramid, and feature fusion is employed to integrate features of different scales, resulting in accurate segmentation of the TMJ in CBCT images. Results The network was trained and evaluated using a clinical image dataset, and the dice similarity coefficient (DSC) was 98.02 ± 0.65 %, the average surface distance (ASD) was 0.0605 ± 0.0229 mm, and the Hausdorff distance (HD) was 0.1907 ± 0.0627 mm. Conclusions The proposed MIP-Net achieved accurate segmentation of the TMJ in CBCT images, which may help to improve the clinical diagnosis and treatment of TMJ-OA.

Design and analysis of metamaterial superstrate and FSS reflector loaded UWB antenna for the detection and localization of breast tumors

Objectives This paper investigates an efficient UWB biosensor that employs monostatic radar-based microwave imaging (RBMI) to identify cancerous cells in human breast tissue. Methods The proposed FSS-loaded lens applicator biosensor assembly combines three designs, including a UWB radiator, a frequency selective surface (FSS) reflector, and a square-shaped split ring metamaterial cell (SSR-M). In Computer Simulation Technology-Microwave Studio (CST-MWS) simulator, a lens applicator (UWB transceiver with SSR-M at its front side) when loaded with an FSS reflector is simulated with a breast phantom (BP), then the system performance is assessed in the context of backscattered parameters, penetration depth (PD), and specific absorption rate (SAR). Experimental validations of the fabricated prototype are captured using the Vector Network Analyzer (VNA). Results/Key findings Signal processing steps are applied over the S 11 signals using MATLAB, to detect the depth/location of the malignant tissue. The proposed system is capable of detecting a minimum tumor size of 4 mm up to a maximum localization depth of 30 mm. The proposed system has an isotropic gain of 3.6 dBi and a SAR of 1.07 W/kg over 1 g of tissue. Conclusions As a result, the proposed biosensor configuration is suitable option for microwave imaging applications.

The investigation on EEG characteristic parameters change when executing cognitive ability tasks under centrifuge G acceleration

Objectives Flight cognitive ability is the core element of guaranteeing flight safety. This study attempts to explore the basic cognitive operation ability of pilots under overload environment, and provides reference for the research on the index screening and monitoring technology methods cognitive operation ability under centrifuge overload condition. Methods A specific cognitive operation ability task is designed, physiological signals such as electroencephalography (EEG) when performing cognitive ability tasks under human centrifuge overload condition are obtained, and the characteristic parameters changes are studied. Results When performing cognitive tasks, both the beta waves and the Theta/Beta Ratio (TBR) parameters in EEG show certain specific changes after the task starts and in the later stage the task. Conclusions Beta wave energy ratio and TBR parameter can better reflect the changes of pilot’s cognition under overload environment, and the joint research of multiple frequency bands and multiple parameters helps to find the parameters and indicators with the potential to become the physiological monitoring index of pilot’s cognitive operation capacity under overload condition more quickly and better. This study lays a foundation and provides a reference for the follow-up selection of physiological monitoring index of a pilot’s cognitive operation ability and the research of monitoring technology on cognitive operation ability under overload condition.

Smartphone-based intelligent system with a wearable throat microphone for real-time screening of swallowing ability

Objectives Accurate and timely swallowing ability screening is essential for effective prevention and treatment of dysphagia. Current clinical imaging methods are often radioactive or invasive, limiting their applicability for routine monitoring. This study aims to develop a smartphone-based intelligent system for real-time screening of swallowing ability using a wearable throat microphone. Methods First, a customized Android application was developed to collect, visualize, and analyze sounds related to swallowing from a wearable throat microphone. Second, a transfer learning model, YAMNet-S, was trained on 4,715 one-second audio segments of coughing, swallowing, and other noises, obtained from 215 healthy participants (Experiment 1). The trained model was then deployed on a smartphone to classify swallowing-related events in real time. Finally, the water swallow test (WST) was conducted by counting swallowing and coughing events within 30 s from 15 simulated patients with self-induced voluntary coughs for clinical validation (Experiment 2). Results The mean accuracy of the trained model in swallowing and coughing events classification is 94.48 and 94.45 %, respectively, and the difference of WST scores in system calculations and expert evaluation was 0.267 (out of 5). Conclusions This smartphone-based intelligent system has the potential to be a comfort, portability and user-friendliness tool for preliminary screening of dysphagia before VFSS/FEES, especially on some situations with limited medical resources, such as community or home-based care.

A computational 3D analysis for assessing bone remodeling following total hip arthroplasty: a longitudinal study spanning six years

Objectives This study aimed to enhance the comprehension of volumetric bone mineral density (vBMD) changes following Total Hip Arthroplasty (THA) by establishing a protocol to (i) precisely locate alterations in the proximal femur in three dimensions and (ii) evaluate these changes over an extended period. Methods Twelve individuals who underwent unilateral THA, using either cemented or uncemented prostheses, were recruited. CT-scans of the proximal femur were acquired at three distinct time points: 24 h, 1 and 6 years post-surgery. Utilizing the acquired data, 3D models of the proximal femur were generated, and a novel algorithm was developed to categorize them into Gruen zones. Comparative analysis of density values among the three sets of scans allowed the calculation of bone density gains/losses for the entire proximal femur and specific regions. Results A lower trabecular bone quantity was observed in the cemented group compared to the uncemented cohort, with discernible differences in vBMD evolution observed in the overall femur and certain Gruen zones. Noteworthy inter-patient variability was evident, ranging from physiological bone remodeling to unexpected increases/decreases in vBMD (e.g.,+340 % after one year). Conclusions This analysis proves to be a valuable tool to understand the long-term vBMD evolution in THA patients.

The impact of physiological load and support conditions on axial implant system testing of locking plates for femoral shaft fractures – a biomechanical analysis

Objectives This study aimed to determine the impact of physiological loading and boundary conditions on the biomechanical performance of a plating system for femoral shaft osteosynthesis via axial implant system testing (IST). Specifically, the effects of rotational load boundary conditions and realistic gait-based load patterns were evaluated to understand their influence on the biomechanical response and failure modes of the implant system. Methods Two test configurations – Fix-Free, featuring a rotational joint, and Fix-Fix, with fixed support at both ends – were subjected to static and cyclic loading. Cyclic testing incorporated sinusoidal and gait-based load patterns, reflecting the physiological axial joint load during walking. In total, 30 test samples (n=30), employed by a bone surrogate and the plate-screw system in bridge-plating state, were tested via axial IST. Results The Fix-Free configuration exhibited significantly lower axial stiffness and load capacity reductions of 60.8 % compared to Fix-Fix in static testing. Under cyclic gait-based loading, both setups experienced progressive screw-plate interface failures, with earlier degradation observed in Fix-Free. Conclusions Findings indicate a strong impact of physiological load patterns and boundary conditions. The results support the need for standards and guidelines for biomechanical testing of osteosynthetic plating systems via IST with universal physiological boundaries.

Detection and analysis of fatigue flight features using the fusion of pilot motion behavior and EEG information

Objectives Pilots are susceptible to fatigue during flight operations, posing significant risks to flight safety. However, single-feature-based detection methods often lack accuracy and robustness. Methods This study proposes a fatigue classification approach that integrates EEG features and motion behavior features to enhance fatigue recognition and improve aviation safety. The method extracts energy ratios of EEG frequency bands ( α , β , θ , δ ), incorporates forearm sample entropy and Euler angle standard deviation, and applies Pearson correlation analysis to select key features. Finally, a Support Vector Machine (SVM) classifier is employed to achieve precise fatigue classification. Results Experimental findings indicate that the proposed method achieves a test accuracy of 93.67 %, outperforming existing fatigue detection techniques while operating with a reduced computational cost. Conclusions This study addresses a gap in current research by integrating physiological and behavioral data for fatigue classification, demonstrating that the fusion of multi-source information significantly enhances detection accuracy and stability compared to single-feature methods. The findings contribute to improved pilot performance and enhanced flight safety by increasing the reliability of fatigue monitoring systems.

Validation and comparison of three different methods for automated identification of distal femoral landmarks in 3D

Objectives Identification of bony landmarks in medical images is of high importance for 3D planning in orthopaedic surgery. Automated landmark identification has the potential to optimize clinical routines and allows for the scientific analysis of large databases. To the authors’ knowledge, no direct comparison of different methods for automated landmark detection on the same dataset has been published to date. Methods We compared 3 methods for automated femoral landmark identification: an artificial neural network, a statistical shape model and a geometric approach. All methods were compared against manual measurements of two raters on the task of identifying 6 femoral landmarks on CT data or derived surface models of 202 femora. Results The accuracy of the methods was in the range of the manual measurements and comparable to those reported in previous studies. The geometric approach showed a significantly higher average deviation compared to the manually selected reference landmarks, while there was no statistically significant difference for the neural network and the SSM. Conclusions All fully automated methods show potential for use, depending on the use case. Characteristics of the different methods, such as the input data required (raw CT/segmented bone surface models, amount of training data required) and/or the methods robustness, can be used for method selection in the individual application.

Chamber design and intensity-modulated ultraviolet-C LEDs for advanced pulsed photonic disinfection

Objectives Contaminated apparatus and surgical tools pose serious health risks. For such purpose, disinfection chambers are employed. However, these systems rely on mercury-based UV lamps which comes with various drawbacks. These limitations have driven interest in Ultraviolet-C Light Emitting Diode (UV-C LED) technology as a safer and more efficient alternative. However, existing studies have not thoroughly explored the impact of varying intensities of pulse width modulation (PWM) on disinfection efficacy. Methods To addess this, the present study designed and tested a LED-based disinfection chamber by employing 4-W 275 nm Surface Mount Device (SMD) LEDs against frequently isolated bacteria. By following prior approach, irradiation time was alternated at 30-s intervals and antibacterial efficacy was assessed through various parameters. Additionally, scanning electron microscopy (SEM) was performed to examine the morphological changes. Results Results indicated that the reduction was significantly influenced (p<0.05) with varying PWM levels (60–100 %), achieving 2.05-log 10 and 1.54-log 10 inactivation against Escherichia coli and Staphylococcus aureus, respectively, upon exposure to 51.24 mJ/cm 2 under maximum exposure settings. Moreover, complete cellular damage leading to bleb protrusion and cell-leakage confirmed the disruption of bacterial DNA. Conclusions In conclusion, UV-LEDs show great potential for disinfection, with efficiency influenced by PWM and dosage.

Correlation of electrogastrography and bioelectric impedance techniques for the gastric motility assessment

Objectives The electrical bioimpedance (EBI) technique has been used to measure gastric motility and emptying parameters. A well-known technique for this purpose is electrogastrography (EGG). No correlation between EGG signal and mechanical motility has been reported. In this study, a direct data comparison of these two techniques was performed. Methods 23 volunteers underwent simultaneous gastric monitoring using EGG and EBI. Signal processing was performed to isolate the slow waves of 0.5–9 cpm. The parameters obtained from 70 % overlapped time slots of 3.5 min, included the dominant frequency and power of the normo-gastric region and the percentage of brady-, normo-, and tachy-gastric slow waves. Results The EGG showed slightly higher values in dominant frequencies, whereas EBI displayed higher variability. High-frequency features were more significant in the EBI, with lower variability, and correlations were found in approximately half of the frequency spectra. Slow waves exhibited poor correlation, but were significant at 95 % of the timeslots. Conclusions Comparing EBI and EGG, global parameters in the normogastric region had slight variances, which may not significantly impact clinical findings. The sensitivity of the EBI to higher frequencies is evident.

Dynamic loading leakage test of dental abutment/implant connections based upon a novel implant system with an abutment switch feature: an in vitro study

Objectives The objective of this study was to evaluate the sealing ability of the implant-abutment interface in a specific implant system in comparison to other implant systems under conditions of increasing dynamic loading. Methods Three different implants and four abutment types were tested: one implant with a conical abutment connection, one implant with a flat abutment connection, and one implant with an abutment switch feature and two different abutment connections. The tests consisted of a phase of cyclic loading followed by a leakage test. The maximum loading force was increased, and the procedure was repeated, until either the implant-abutment connection failed, or a leakage was detected. Loading criteria were modified based on the ISO standard 14801:2016. Results The conical implant abutment connections exhibited fracture prior to leakage at varying failure forces and loading cycles, whereas the platform abutment type showed leakage or fracture at a relatively low force. The two different abutment types used on the same implant system showed extremely different results. Conclusions Literature shows similar high chewing forces mostly only for healthy subjects. For patients with dental implants, leakage is still preferable to fracture. In conclusion, the test showed that some implants with a conical abutment are more resistant to leakage and may be preferable if higher chewing forces are expected.

How does research output and impact in medical informatics vary among EU member states? – A bibliometric analysis

Objectives This study evaluates how research output and impact in medical informatics vary among EU member states before and during the COVID-19 pandemic by analyzing publication volume, impact metrics, collaboration patterns, and open-access trends. It seeks to identify regional disparities, highlight key research themes, and provide insights for researchers, the public, and policymakers to promote equitable access, collaboration, and investment in medical informatics across the EU. Methods A bibliometric analysis was performed using Clarivate Web of Science and InCites databases, encompassing 6,620 articles from 47 medical informatics journals published between 2018 and 2022. Metrics such as cumulative impact factors, article counts, and collaboration trends were analyzed. Results Our analysis identified substantial regional disparities in research output and impact. Western European countries, including Germany, the Netherlands, and Spain, consistently led in article volume and cumulative impact factors, while Eastern European countries showed lower engagement. Collaboration metrics revealed that 66 % of publications involved international partnerships, showcasing strong cross-border cooperation within the EU. Conclusions This study highlights the uneven distribution of research productivity in medical informatics across the EU. The findings underline the importance of international partnerships and equitable access to research in advancing medical informatics and addressing evolving healthcare challenges.
