Medical staff are expected to cooperate with other professions and agencies in helping the young human in achieving the goal of becoming a healthy, well- functioning adult that expresses her/his maximal potential. Achieving this goal should be cost-effective. Cost includes not just the economic burden but also psychosocial determinants such as emotional disruption, stress, living at risk, malfunctioning, and dependency. Acknowledging the risks and the expected achievements at each age are useful in analyzing the failure of community health programs and in planning preventive modalities and needed remedies.

Background: While The Bahamas have significantly reduced poor reproductive health outcomes among adolescents and emerging adults, data indicate that youth are engaged in sexual risk behaviors. Substance use has been linked to increased risk for HIV and sexually transmitted infections in other contexts. There are limited data on Bahamian youth in relation to consumption of alcohol and marijuana use and engagement in sexual behaviors. Objective: This study aimed to assess potential relationships between alcohol and marijuana use and engagement in sexual behavior among government secondary school students in New Providence, The Bahamas. Materials and methods : Total sample size was 2572, and about 56% of respondents were female. Mean age was 14.2 (SD 2.7 years). Cross-sectional data came from a baseline survey conducted as part of a longitudinal randomized controlled evaluation of a school-based HIV prevention and reproductive health program in New Providence. Results: Overall, 46.5% (519) males and 44.8% (652) females reported alcohol consumption; 7.3% (82) males and 1.7% (25) females reported use of marijuana in the last 6 months. About 43% (477) male respondents and 16% (231) female respondents reported ever having vaginal sex. Logistic regression analysis indicates that increased likelihood of engaging in sex during the past 6 months is associated with being older, male, and consuming alcohol and marijuana. Conclusion: These data provide a ‘global correlation’ between substance use and engagement in sexual behaviors among Bahamian adolescents. Longitudinal research is needed to assess event specific risks and identify mediating and moderating factors. These findings indicate the importance of integrating reproductive health and substance use education.

Background : Endothelial dysfunction predicts adult cardiovascular disorder and may be associated with negative emotions in adolescents. This study was conducted to determine if hopelessness, hostility, and depressive, anxiety, or conduct disorders were associated with compromised endothelial function and whether those associations were mediated by health risk behaviors. Methods : Endothelial function, assessed through brachial artery reactive hyperemia, was measured in a psychopathology enriched sample of 60 15–18-year-old girls. The correlations between hopelessness, hostility, and depressive, anxiety, or conduct disorders and the percent change in forearm vascular resistance (PCFVR) were measured. Possible mediation effects of health risk behaviors were tested. Results : Hopelessness was negatively associated with PCFVR, controlling for race and body mass index. Conduct disorder without any anxiety disorder was associated with better endothelial function. The other negative emotions were not associated with PCFVR. Risky health behaviors were associated with conduct disorder and hopelessness, but not with PCFVR, so there was no evidence of mediation. Conclusion : The main finding was that hopelessness in adolescent girls was associated with endothelial dysfunction. This may indicate that when present, hopelessness places a girl at risk for later cardiovascular disease, whether she has a psychiatric disorder or not. Possible mechanisms for this finding are examined and the surprising finding that conduct disorder is associated with better endothelial function is also discussed. Suggestions for future research are presented.

Background: The World Health Organization recommends HIV testing and counseling (HTC) for all adolescents living in countries with generalized HIV epidemics. In Botswana, HIV prevalence among adolescents 15–19 years is 3.7% and among pregnant adolescents is 10%. We describe the proportion and characteristics of secondary school students who have accessed HTC. Methods: A multistage sample survey was conducted among students in Botswana’s public secondary schools in 2010. The survey was self-administered using a personal digital assistant device. The HTC rate was estimated using self-reported history of HIV testing. Results: Of 1,632 participants, 52% were girls, 43% aged below 16 years, and 27% had ever had sexual intercourse. Most (81%) students knew where to get tested for HIV. Overall, 2.2% of students were HIV positive by self-report. The HTC rate was 23% overall, 34% among students who had ever had sexual intercourse, and 45% among students who had sexual intercourse in the past 12 months. Being pregnant or having made someone pregnant and having had sexual intercourse in the past 12 months were associated with having been tested for HIV among students who had ever had sexual intercourse. Discussion: Overall, the HTC rate was low, and the self- reported HIV prevalence was high among secondary students in Botswana. Most sexually active students have never been tested for HIV. Health communications efforts for adolescents that increase demand for HTC, routine opt-out HIV testing in healthcare facilities, and school-based HIV testing are needed as part of a national HIV prevention strategy.

Aim: Although characteristics of health-promoting schools are mentioned in the World Health Organization guidelines, different countries need to design more details of indicators for assessing these schools according to their social and cultural context. The aim of this study was to investigate characteristics of health-promoting schools from Iranian adolescent girls’ point of view. Materials and methods: In this cross-sectional study, 2010 middle school and high school female adolescents were selected from randomly selected schools in Mazandaran province, Iran. They completed a self-completion questionnaire around their views about characteristics of health- promoting schools. Data were analyzed using descriptive statistics and an independent t-test. Results: It is revealed that from Iranian adolescents’ point of view the most important feature of health-promoting schools was the schools with no stressful exams and where notices are kindly given to students for their mistakes. Conclusion: The results suggest that there is a need for more measurable standards of health-promoting schools based on the socio-cultural context of both developing and developed countries.

Background: Abortion complications constitute 13% of maternal deaths worldwide and 40% of maternal deaths in Nigeria, while 80% of patients with abortion complications are adolescents. Aims: Assessing the adolescents’ knowledge and attitude and the determinants of their sexual activities and contraceptive usage. Methods: This was a cross-sectional study conducted among high school students of Ogbomoso – a semi-urban settlement. Stratified multistage randomization was used to select one high school from each of five local government areas of the study site. Students were interviewed with pretested questionnaire to sought information on demographic status, family type, knowledge of sexuality, and contraceptive usages. Results: The age range of the respondents was 10–19 years with mean±SD age 13.7±1.9 years. Among them, 6.5% were sexually active. Of the respondents, 56.8% scored good knowledge about sexuality and contraception and 57.5% respondents had good attitude to contraception. Age (p=0.004) and father’s education (p=0.001) were factors associated with knowledge about sexuality and contraception, while mothers’ occupation (p=0.02) and fathers’ occupation (p=0.001) and education (p=0.04) were significantly associated with positive attitude toward contraception. Conclusion: This study revealed that a significant percentage of the studied adolescents had poor knowledge and attitude toward contraception, and many of the sexually active respondents had inaccurate knowledge on the use of contraceptives and where it could be obtained.

Background: Pornography can affect the lifestyles of adolescents, especially in terms of their sexual habits and porn consumption, and may have a significant influence on their sexual attitudes and behaviors. Objective: The aim of this study was to understand and analyze the frequency, duration, and perception of web porn utilization by young Italians attending high school. Materials and methods: A total of 1565 students attending the final year of high school were involved in the study, and 1492 have agreed to fill out an anonymous survey. The questions representing the content of this study were: 1) How often do you access the web? 2) How much time do you remain connected? 3) Do you connect to pornographic sites? 4) How often do you access pornographic sites? 5) How much time you spend on them? 6) How often do you masturbate? and 7) How do you rate the attendance of these sites? Statistical analysis was performed by Fischer’s test. Results: All young people, on an almost daily basis, have access to Internet. Among those surveyed, 1163 (77.9%) of Internet users admit to the consumption of pornographic material, and of these, 93 (8%) access pornographic websites daily, 686 (59%) boys accessing these sites perceive the consumption of pornography as always stimulating, 255 (21.9%) define it as habitual, 116 (10%) report that it reduces sexual interest towards potential real-life partners, and the remaining 106 (9.1%) report a kind of addiction. In addition, 19% of overall pornography consumers report an abnormal sexual response, while the percentage rose to 25.1% among regular consumers. Conclusion: It is necessary to educate web users, especially young users, to a safe and responsible use of the Internet and of its contents. Moreover, public education campaigns should be increased in number and frequency to help improve knowledge of Internet-related sexual issues both by adolescents and by parents.

Objective: We made anthropometric measurements to compare body proportions of university students and to investigate the tendency of the students with more developed bodies in choosing profession for university education. Methods: A total of 204 male students [group 1: students of School of Physical Education and Sports” (SPESp) (n=88), group 2: students of Faculty of Fine Arts (FFArts) (n=59), and group 3: students of Faculty of Medicine (FMed) (n=57)] were included, and anthropometric measurements were performed. Results: “Height”; “chest circumference”; “difference of inspiration and expiration of chest circumference” values of the SPESp students were higher than the others (FFArts and FMed). In the SPESp students, chest circumference and chest expansion capacity by inspiration developed more. This shows the positive impact of sport on respiratory functions. Additionally, the legs are longer in students engaged in regular sports. Conclusion: Exercise is important for all subjects in terms of body development, growth, and oxygenization and to decrease cardiovascular risk factors. For the development of the ideal body anthropometric rates, obesity prevention, and for the growth of healthy generations, governments should give importance to sports and take incentive measures to increase children and young people’s interest in the sport.

Objective: Cyberharassment/cyberbullying is a global problem that has been inadequately investigated in developing countries. In this paper, we present findings on the prevalence and predictors of perpetration of cyberbullying among in-school adolescents in Oyo state, Nigeria. Methods: A total of 653 students were selected via multi-stage sampling. Information on history of perpetrating harassment via an electronic medium in the 3-month period preceding the survey was obtained. Results: Respondents’ mean age was 14.2±2.2 years and 51.3% were females. All respondents had personal mobile phones and about half had Internet access. About 40% accessed the Internet every day while about 48% accessed it at least once to several times a week and <5% accessed it about once every 2 weeks. One hundred and fifty-six (23.9%) had harassed someone electronically, 260 (39.8%) had been victimized, and 137 (21.0%) were both victims and perpetrators. Common modes of harassment were via phone calls 99 (63.5%), chat rooms 70 (44.9%), and text messages 60 (38.5%). Students who had been victims of cyberbullying (OR=21.76, 95% CI=12.64–37.47) and those with daily Internet access (OR=2.32, 95% CI=1.28–4.19) had significantly higher Oods of being perpetrators. Conclusion: About a quarter of students were perpetrators of cyberbullying, and the correlates of perpetration were history of cyber victimization and daily Internet access. Intervention programs must be instituted for victims as well as frequent users of the Internet to curb the problem in the study area.

Background: Instrumentation with established reliability and validity is not yet routinely utilized to assess readiness for transition from pediatric to adult care for youth and young adults with chronic conditions, including sickle cell disease (SCD). Objective: The aim of this study was to develop a SCD specific readiness for transition assessment tool. Subjects: Fifty-seven youths with SCD, ages 15–21 years, completed the initial version of the Transition Intervention Program – Readiness for Transition ( TIP-RFT ) assessment; 113 youths/young adults with SCD, ages 14–26 years, at two distinct sites of care completed a refined version of the TIP-RFT . Methods: The TIP-RFT was constructed based on a literature review, provider and patient consensus and assessed domains including knowledge and skills in medical self-care, social support, health benefits and independent living and educational/vocational skills. We used principal components factor analysis to evaluate TIP-RFT responses and assessed differences in TIP-RFT scores in relation to age, gender, sickle cell diagnosis and site of care. Results: The original TIP-RFT , which had demonstrated face validity, was reduced from 56 to 22 items. The revised instrument consisting of four subscales demonstrated good internal consistency reliability and construct validity. Conclusion: Our results support that the TIP-RFT is a valid and reliable tool for the assessment of transition readiness for youths with SCD. The TIP-RFT assessment can guide interventions to improve transition readiness and can provide a foundation for future research on other variables that might be associated with transition readiness.

Purpose: To determine the sexual behavioral pattern, consequences, and adopted solutions among senior secondary schools students in Ojo military barracks, Lagos. Materials and methods: This was a cross-sectional study of 400 senior secondary schools students in Ojo military barracks, Lagos, selected using a multistage sampling technique. Data was collected using pre-tested, self-administered semi-structured questionnaires. Data was analysed using statistical package for social sciences version 17. Tests of statistical significance were carried out using χ 2 -test, and a p-value of <0.05 was considered significant. Results : The majority of them 391 (97.8%), were in the age group (10–19 years) while the mean age was 15±2.4 for males and 15±2.2 for females, respectively. One hundred and fifty four (38.5%) of the respondents have had penetrative sexual intercourse and were influenced mainly by peer pressure (p=0.0000). Their age at first sexual intercourse ranged between 10–19 years. Ten (6.5%) of the respondents (all males) had their sexual debut with a commercial sex worker. More girls 41 (56.2%) than boys 12 (14.8%), were forced into sexual intercourse (p=0.0000). Over half of the sexually active respondents had at least two sexual partners for either sex. Conclusion: Findings from this study have shown that young people living in the barracks indulged in sexual activity at very early ages and engaged in risky sexual behavior. We therefore recommend an improved multi-sectoral approach in reproductive health and sexually transmitted infections including HIV/AIDS education.

Introduction: Tobacco use, which begins in adolescence and childhood and continues in later life, is the major avoidable risk for non-communicable diseases and death in the world. Self-reports have frequently been used to estimate smoking prevalence and health consequences. This study explores the validity of self-reports of smoking behavior among schoolchildren in Tunisia. Materials and methods: This study was conducted in March 2014 among a sample of 147 schoolchildren randomly selected. Data concerning the smoking habit were collected by a questionnaire designed for the purposes of this work. Then, exhaled CO, a biochemical marker of smoke exposure, was measured using piCO+ Smokerlyzer ® breath CO monitor among participants. Sensitivity and specificity of self-reports were calculated. Results: The prevalence of reported smoking was 9.5% with 16.7% and 1.7% respectively among boys and girls. Their mean age was 14.5±1.28 years old. When considering 4 ppm as the cut-off level of breath CO, sensitivity and specificity of self-reports were 100% and 93.7%, respectively. But at a breath CO cut-off of 3 ppm, self-reporting was 62.5% sensitive and 93.5% specific. Conclusion: According to our findings, we suggest that self-reports can be considered as a good tool to be used with a reasonable confidence to assess the smoking status.

Purpose: To review our institution’s experience with frostbite injury secondary to “salt and ice challenge” (SIC) participation. Methods: We conducted a retrospective analysis of intentional freezing burns from 2012 to 2014. Demographics, depth and location of burn, total body surface area of burn, treatment, time to wound healing, length of stay, complications, and motives behind participation were analyzed. Results: Five patients were seen in the emergency department for intentional freezing burns that resulted from SIC (all females; mean age: 12.3 years; range age: 10.0–13.2 years). Mean total body surface area was 0.408%. Salt and ice was in contact with skin for >10 min for two patients, >20 min for two patients, and an unknown duration for one patient. Complications included pain and burn scar dyschromia. Four patients cited peer pressure and desire to replicate SIC as seen on the Internet as their motivation in attempting the challenge. Conclusion: SIC has become a popular, self-harming behavior among youths. Increased public education, and provider and parent awareness of SIC are essential to address this public health concern.

Within the contemporary culture of consumption, school-aged adolescents, though neither waged nor salaried producers, are nevertheless treated by the media and the advertisers as if they are active consumers who are engaged in the project of the self. For those adolescents who lack the financial resources to ‘buy into’ this culture, anxiety may ensue. In order to ease this anxiety, and to acquire social status, some – not all – may make the ‘rational’ ‘consumer’ choice to engage in risky health-related behaviour. In situ ethnographic research is needed in order to complement and inform the existing survey-based evidence on the relationship between economic status and health-related behaviour among school-aged adolescents as they deal with the pressures of consumerism.

Objective: To determine whether early parenthood is associated with the onset of overweight/obesity during adolescence. Methods: Weight status changes between ages 16 and 21 years were measured in 270 Chilean youths. Parenthood by age 21 was assessed by interview. Results: Sixty-three youths became overweight/obese between ages 16 and 21 years (23%), and 24% (n=65) of the total sample were parents by age 21. Bearing a child by age 21 was associated with a two-fold risk of becoming newly overweight or obese (OR=2.6, CI: 1.1, 5.9, p<0.05). Earlier internalizing problems were also associated with the development of overweight/obesity in young mothers. Conclusion: Weight status changes from normal to overweight or obese were more likely to occur among young parents than non-parents. This has implications for adolescents’ future health given their likelihood of having subsequent pregnancies and the known risks of increased weight at each pregnancy.
