Although laboratory tests are the most used diagnostic investigations for screening, diagnosing, prognosticating and therapeutic monitoring of most human diseases, laboratory medicine is currently seen as many other economic industries by some policymakers and administrators, and is hence subjected to scale economy and assessed accordingly, despite the incremental clinical value that laboratory tests can generate. According to a genuine economic perspective, the impact of diagnostic testing on a healthcare budget is lower than 2.5%, whilst its profitability is over 100%, a net profit margin over 7-fold larger than whatever other human industry. Even more importantly, the impact of laboratory tests on clinical outcomes is now clear and virtually incontestable, as their use will improve clinical pathways much more than any other diagnostic investigations. The many ongoing attempts to downsize the importance of laboratory medicine as costs centers, or even the concept that public laboratory services can be safely eliminated or outsourced to external private professional organizations, shall hence be challenged. Laboratory medicine not only is vital to patient care and patient flow, and will remain so for many years to come, but is also a valuable economical resource for the healthcare facilities.

Background Interleukin (IL)-13 is a regulatory factor of tissue remodeling and is involved in the pathogenesis of pulmonary artery hypertension (PAH). However, the implications of IL-13 in PAH remains uncertain. This article aims to describe the current knowledge on production and function of IL-13 and its receptors in the mechanisms of PAH. Content The study materials of this article were based on comprehensive literature retrieval of publications of IL-13 in PAH. These study materials were carefully reviewed, analyzed and discussed. Summary IL-13 levels in blood and lung tissue were elevated in both animal models of PAH and patients with PAH in comparison to non-PAH controls. Types I and II IL-13 receptors participate in pulmonary artery remodeling through signal transducer and activator of transcription (STAT)6 or through phosphatidylinositol 3-kinase (PI3K), STAT3 and mitogen activated protein kinase (MAPK) pathways. Oxidant, arginase 2 (Arg2) and hypoxia-inducible factor 1α are involved in the proliferation of pulmonary artery smooth muscle cells. Outlook Types I and II IL-13 receptors play an important role in the IL-13 signaling by STAT6 via Janus kinase kinases, and by PI3K, STAT3 and MAPK pathways, respectively. Alternative pathways, including oxidant, Arg2 and hypoxia-inducible factor 1α might be also involved in the pathological process of PAH development. Investigational therapies by inflammatory suppression or thrombolytic and anticoagulant agents could inhibit intimal hyperplasia of the pulmonary arteries and suppress pulmonary vasculature remodeling. Drug research and development oriented by this hypothesis would confer benefits to the treatment of PAH.

Background Sexually transmitted infections (STIs) represent a growing relevant public health problem. Early and accurate STI diagnosis is capable of preventing the spread and severe complications of curable STIs through pathogen adapted antibiotic treatment regimens. Limitations of current STI diagnostic tools are the lack of simultaneous pathogen detection and result quantification. Methods A prospective analysis of clinical specimens (n=100, from 82 symptomatic patients) from different sites of infections was performed. All samples were processed with multiplex real-time polymerase chain reaction (PCR) assay Anyplex™ II STI-7, using thermal cycler CFX96™. The Anyplex™ II STI-7 detection assay covers Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG), Mycoplasma genitalium (MG), Mycoplasma hominis (MH), Ureaplasma parvum (UP), Ureaplasma urealyticum (UU) and Trichomonas vaginalis (TV). STI-7 results were compared to standard methods and transferred to the attending physician for treatment adjustment. Results Compared to performed standard method results the assay achieved sensitivities ranging from 90.9% to 100%, respectively, and specificities of 100%, with negative predictive values (NPV) ranging from 98.5% to 100%, respectively, and positive predictive values (PPV) of 100%. The Anyplex™ II STI-7 detection assay measured a Cohen’s kappa of 1.00 for UU/UP, MH, and MG and a Cohen’s κ agreement of 0.95 and 0.96 for NG and CT, respectively. Conclusions The Anyplex™ II STI-7 assay can easily be introduced into the microbiological laboratory work flow due to its short hands-on-time and PCR mutiplexity. The simultaneous detection of seven STI pathogens provides a comprehensive profile for each patient, enabling clinicians to decide on best treatment options, decreasing antibiotic misuse and infection spreading risk. The semi-quantitative results enables clinicians to gain a complete package of diagnostic information including disease diagnosis, disease degree severity and treatment monitoring, although. Further clinical studies on this topic are needed.

Background: Helicobacter pylori inhabit the gastric mucosa of humans and are associated with several gastrointestinal diseases which include gastric cancer, peptic ulcer, chronic gastritis and gastric mucosa-associated lymphoid tissue lymphoma. Helicobacter pylori exhibit a high degree of genetic variability and are associated with its epidemiological, pathological characteristics and dynamics of transmission. The objective of the study was to determine the prevalence and genetic heterogeneity of H. pylori isolated from endoscopic biopsy samples from a Chinese population. Methods: Gastric biopsy samples from 86 patients (males, 55; females, 35) who presented to the endoscopic section for various gastrointestinal abnormalities were collected. The samples were subjected to a real-time polymerase chain reaction (PCR) and microbial culture for the isolation of H. pylori . Further, the isolates were subjected to randomly amplified polymorphic DNA (RAPD) and restriction fragment length polymorphism (RFLP) analysis. Results: Of the 86 gastric biopsy samples, 61 (70.9%) samples were positive for rapid urease test and 37 (43%) samples (28 from male and nine from female) grew H. pylori . Among the biopsy samples subjected to real-time PCR, 39 (45.3%) samples were found to be positive for H. pylori. The RAPD analysis yielded 15 different patterns (four to 17 different sized fragments per strain). The phylogenetic analysis of RAPD yielded 22 clusters at a similarity level ranging from 63% to 100%. RFLP analysis yielded nine different patterns (two to six different sized fragments per strain). Two major restriction patterns were identified, of which 14 (37.8%) strains forms the most common pattern (genotype I) followed by five (13.5%, genotype II) strains with an intra-strain similarity of 100%. Conclusions: The overall prevalence of H. pylori was 45.3%. Despite reports on the declining trend in the prevalence of H. pylori infections, our prevalence rate was still higher than those reported from other developed countries. However, further studies involving a large sample size and covering more regions of China is highly warranted.

Background Retinol-binding protein 4 (RBP4) and cystatin C are regarded as novel metabolic risk markers. Therefore, we aimed to examine which one of these biomarkers better correlates with metabolic syndrome (MetS) in a cohort of postmenopausal women. Methods A total of 129 postmenopausal women (among which 62 women had MetS) were recruited in this cross-sectional study. MetS was diagnosed according to the International Diabetes Federation criteria. Results Cystatin C and RBP4 levels were significantly higher in women with MetS, compared to those without MetS (p=0.011 vs. p<0.001, respectively). A significant difference in the proportion of women with and without MetS across cystatin C and RBP4 quartiles was observed (χ 2 =5.1, p=0.025, and χ 2 =11.1, p=0.001, respectively). Logistic regression analysis revealed a borderline significant relationship between cystatin C and MetS (p=0.066), but this significance disappeared after adjustment for age, inflammation level and duration of menopause (p=0.221). On the contrary, a significant relationship between RBP4 and MetS was observed not only without adjustment (p=0.009), but also even after adjustment for age, inflammation level and duration of menopause (p=0.006). Conclusions RBP4 better correlates with MetS than cystatin C in postmenopausal women.

Background Cardiac troponins are the recommended markers for the detection of acute myocardial infarction (AMI). There is a controversy regarding their decision limits. In this study, our objective was to reveal the cut-off values of high-sensitive troponin T (hsTnT) for AMI diagnosis in our population and to examine the effect of age and gender on hsTnT cut-off values. Methods Patients who presented to the emergency department (ED) with chest pain were selected, and only those patients admitted during the first 3–6 h of symptom onset were included in the study. Results A total of 484 men and 182 women were included. A total of 355 (279 men/76 women) patients were diagnosed with AMI. The cut-off values of hsTnT were found to be 17 ng/L and 16 ng/L, for males and females, respectively. The cut-off values of hsTnT were detected to be significantly higher in men over 40 years of age (24 ng/L) than in men less than 40 years of age (10 ng/L). Conclusions The cut-off value for the hsTnT test for AMI was slightly lower in females than in males. The cut-off levels of hsTnT for the diagnosis of AMI were found to be significantly higher in men over 40 years of age than in men less than 40 years of age.

Background This report investigates an unusual total testosterone result in a middle-aged female. Case presentation A 40-year-old female patient was found to have high serum total testosterone of 30.82 nmol/L on a Beckman Coulter UniCel ® DxI800 automatic chemiluminescence immunoanalyzer without appropriate clinical appearance. Heterophile antibody interference was considered for elevated testosterone and investigated; reanalysis of the original serum sample and liquid chromatography with tandem mass spectrometry (LC-MS/MS) were performed. Reanalysis of the original serum sample using Roche ® and Siemens ® immunoassays both gave a normal total testosterone level. Beckman Coulter ® also excluded the possibility of interference by heterophile antibodies. Finally, LC-MS/MS showed that the total testosterone level was in the normal range. Conclusions This report highlights the importance of ruling out the interfering factors for false high values of the analyte in laboratories.
